NDIS Plan-Managed

Submit the 'Apply for Plan-Managed Account Form' below. Then create an account and you can immediately place an order. Choose the 'Request Invoice (Wholesale, NDIS Plan-Managed, NDIA purchase above $1500)' option at checkout. We will email you and your plan manager an NDIS-compliant PDF invoice.

Heads up: if your cart contains items that are NDIS-covered and items that aren't the invoice will require you to personally pay for the non-NDIS items.

Plan-Managed Form

NDIS Self-Managed & NDIA Managed

You manage your own NDIS funds and claim reimbursements directly. Simply submit the form, create an account and you can shop normally. You will receive your NDIS-compliant invoice via email which you can then submit the invoice to the NDIA for reimbursement.

NDIA Managed: The NDIA manages your plan and pays providers directly. For consumables under $1,500, you can shop normally and claim reimbursement just like self-managed participants.
